Match Report / Score Sheet
After seeing a team use a hand drawn match report sheet, Rob Joubert began researching whether or not a freely available match report sheet could be downloaded from the Internet. When he couldn't find one he teamed up with our publisher, Cris Maloney, and together they created a form that's free to use. It is posted here. Please download a copy, print it, use it, and distribute it to everyone who needs it.

USA Field Hockey has released the 2011 version of its annual comparison table of the Rules of Hockey and the modifications that are used within the USA to govern NFHS (National Federation of State High School Associations) and NCAA (National Collegiate Athletic Association) competitions.

Umpire Evaluation Form
UmpireHockey.com has updated its popular assessment form used for game performance evaluation. New this year is a form dedicated to evaluator instruction, are scoring instructions and a tip for umpire coaches on how to use the form to remember to give positive feedback first. A second form, with instructions for the evaluator is also available. [The first verision of this form for public release was published here on August 28, 2005. Earlier versions had been in use since August 2003.]

Whistle Counter
Are there too many whistles in hockey? Fans that don't know the rules certainly think so. UmpireHockey.com has created a form to collect data on the number of times each umpire blows their whistle and at what point during the game. Analysis of submitted forms could provide interesting information for trend reporting.

Competition Surface Safety Form
Here is a form our readers in the USA might find of interest. It allows umpires to quickly fill out and fax a field safety report to the appropriate state association (fax numbers are included on the form) when a competition site isn't prepared to the specifications detailed in the National Federation's field hockey rule book.
