Whistle Counter
Are there too many whistles in hockey? Fans that don't know the rules certainly think so. UmpireHockey.com has created a form to collect data on the number of times each umpire blows their whistle and at what point during the game. Analysis of submitted forms could provide interesting information for trend reporting.

Competition Surface Safety Form
Here is a form our readers in the USA might find of interest. It allows you to quickly fill out and fax a field safety report to the appropriate state association (fax numbers are included on the form) when a competition site isn't prepared to the specifications detailed in the National Federation's field hockey rule book.
