Umpire Team Evaluation Form
Most umpire evaluations are completed for only one umpire and, by a trained evaluator. This form, instead, provides a coach, reporter, or everyday fan with the opportunity to evaluate a team of umpires. Very few of the questions are required so, if all you want to do is record how long the timeouts were in the game you watched/are watching, skip over the other questions. You can complete it real-time by opening it on your smartphone at a game. Alternately, you can print out a copy of the form, put it on a clipboard, and bring it to a game. The form will help you learn what to look for in order to evaluate an umpire.
