Helpful Documents UmpireHockey.com has updated its popular assessment form used for game performance evaluation. New this year is a form dedicated to evaluator instruction, are scoring instructions and a tip for umpire coaches on how to use the form to remember to give positive feedback first. A second form, with instructions for the evaluator is also available. [The first verision of this form for public release was published here on August 28, 2005.
